EMERGENCY services were called to Lawrence Hargrave Drive at Stanwell Park just before 3.30pm on Thursday last week, after a Yamaha motorcycle and a Kenworth Prime Mover crashed head on.
The 16-year-old male rider was seriously injured. He was treated by paramedics before being airlifted to St George Hospital in a critical condition.
The truck driver was uninjured and taken to Wollongong Hospital for mandatory testing.
Officers from Wollongong Traffic and Highway Patrol Command attended and are investigating the circumstances surrounding the crash.
Police have been told a second motorcyclist was seen travelling north along Lawrence Hargrave Drive prior to the crash and failed to stop.
Anyone with information, including dash-cam footage, is urged to come forward.
